Medical Cannabis Patient Program (MCPP)

Please read the MCPP participation Guidelines below before you submit your application.


Have your healthcare professional complete the Healthcare Professional Written Certification form that MUST be submitted with your patient application.

Complete the Medical Cannabis Patient Application

Gather the following documents to be submitted with your qualifying patient application (if applicable)*

QUALIFYING PATIENTS MUST HAVE

  • Illinois Driver’s License or State ID
  • Proof of residence documentation
  • 2×2 Passport-Style photograph
  • Credit or debit card for state application fee
  • Completed Caregiver application (if applicable)

Submit the application and required documents to the Illinois Department of Public Health (IDPH) – via mail or online at https://medicalcannabispatients.illinois.gov

To qualify for a patient registry identification card, a qualifying patient must:

  • Be a resident of the State of Illinois
  • Have at least one of the approved 52 qualifying debilitating medical conditions, or a terminal illness diagnosis
  • Be at least 18 years of age*
  • Not hold a school bus permit, CDL, or be an active duty law enforcement officer, correctional officer, correctional probation officer, or firefighter

Applicants should discuss the use of medical cannabis with their physician prior to beginning an application. All applicants, except of Veterans receiving care at a VA facility, must have their physician submit a written certification for the use of medical cannabis.

Patients must have an ongoing bona-fide medical relationship with a physician before being certified. The physician remains responsible for the ongoing care of the qualifying patient.

Ask your healthcare professional to complete the Healthcare Professional Written Certification Form and return it to you to include it with your application. You must see your physician no more that 90 days before you submit your application.

Persons who apply online for MCPP will be issued an eRegistry Card to enter the dispensary within 24 hours of submitting the qualifying application.

Veterans receiving health care at a VA facility do not need to provide a physician written certification but must instead provide medical records from the VA facility for the last 12 months. Use VA form 10-5345 to request these records (see page 1 of the Qualifying Patient Application for details).

Photograph requirements:

  • Taken in the last 30 days
  • Taken against a plain, white or off-white background or backdrop
  • In natural color (Do not use a filter)
  • Full-face view directly facing the camera with a neutral facial expression and both eyes open
  • At least 2 inches by 2 inches in size
  • It’s recommended you use a passport photo vendor to ensure the photograph meets these requirements.

Submit 2 proofs of residency unless your government issued ID matches your application, then submit one additional proof of residency. (i.e.: current Bank Statement, Utility Bill, Insurance Policy = current coverage, W-2 from most recent tax year, pay stub of electronic deposit slip, Valid Voter registration, credit cart statement, current military ID card, property tax bill, rental/lease agreement, deed/title, mortgage…)

Non-refundable application fee – Check or Money order to Illinois Department of Public Health if mailing in the application. Mail the application and required documents to: Illinois Department of Public Health of Medical Cannabis 535 West Jefferson Street Springfield, Illinois 62761-0001.

Credit or debit cards are acceptable for online applications only. Submit online applications at: https://medicalcannabispatients.illinois.gov. If applying for a reduced fee include a copy of your Social Security Benefit Verification Statement for SSI or SSDI; Veterans will need a copy of their DD-214.

OPIOID ALTERNATIVE PILOT PROGRAM (OAPP)

Have your healthcare professional complete the online Healthcare Professional Written Certification From (PC)

Select a dispensary and register for OAPP online

Receive emailed temporary password (patient must change password)

Sign in to ICTS and select Physician Certification

Patient will receive email confirmation of eRegistry card information

Bring required documentation to dispensary

QUALIFYING PATIENTS MUST HAVE

  • Illinois Driver’s License or State ID
  • Prood of Address documentation
  • 2×2 Passport-Style photograph
  • Credit or debit card for $10 state application fee

An Illinois Licensed Physician must certify an individual has a medical condition for which an opioid has been or could be prescribed based on generally accepted standards of care.

Qualifying patients for OAPP must be at least 21 years old and be a resident of Illinois.

  • Registered patients are eligible to purchase 2.5 ounces of medical cannabis every 14 days.
  • No caregiver provisions for OAPP
  • People with CDL-driver’s license or a school bus driver permit cannot apply.
  • Patients must have a valid email address for OAPP

Data from the Illinois Department of Public Health (IDPH) indicates opioid deaths in Illinois increased 13 percent from 2016 to 2017. The long-term goal of this program is to reduce opioid deaths.

OAPP:

An Illinois licensed physician must certify an individual has a medical condition for which an opioid has been or could be prescribed based on generally accepted standards of care.

  • Physician Certifications are valid for 90 days but can be renewed to allow patients to continue accessing medical cannabis

A patient may select his/her Dispensary and register for the OAPP after an in-person office visit, and once the physician certification has been entered online.

  • Patients can register online on the Illinois Cannabis Tracking System (ICTS) https://etk.icts.illinois.gov
  • Patients must register within 30 days of the Physicians Certification (PC)
  • Patients must have a valid email address for OAPP.
  • Patients may come in to the dispensary for help with the online registration process or visit a participating local health department.

Patients will. Receive a temporary password to his/her email address from the ICTS.

  • ICTS will verify whether patient is registered already with MCPP.
  • Patient will use temporary password to log into ICTS
  • ICTS will match patient registration to physician certification.
  • Patient will select PC to download to registration.

The OAPP registered patient will receive an email with his/her registry card information.

  • A dispensary agent will verify registration in Stated seed-to-sale tracking system (BioTrack Traceability) each time patient visits dispensary.

OAPP Re-enrollment – within 30 days of the patients OAPP registration expiration date, the patient may contact his/her physician for an appointment to review their continued participation in OAPP.

  • Requires  in-person office visit
  • New PC submitted online by doctor
  • Updated registration and $10 fee. Must have debit or credit card for he fee.
